PROGRAM DESCRIPTION: Supported Access to Primary Health Care is a wraparound worker-supported process for linking Lunenburg Country youth, adults and families with Primary Health Care who may not otherwise choose to access the healthcare system.

PROGRAM SUPPORTS: Trauma-informed access to medical consultation and examination site at Freeman House; Follow up support provided as needed; Preventative individualized primary healthcare access in situations that may escalate into emergencies; Gender-based competency (sexual and reproductive health support including access to abortion services); 2SLGBTQIA+ support (for non-binary and/or transitioning persons); Confidential access; Non-stigmatizing harm reduction supports; Safe supplies.
